A. To submit your chapter membership orders:

1.  Send, a check or money order for payment of national dues made payable to "Phi Lambda Upsilon" for each initiate and any additional items ordered directly to the PLU National Treasurer:  Dr. Don Land, Department of Chemistry, UC Davis, One Shields Ave., Davis ,CA  95616. National dues are $35 (as of July 1, 2005) for a lifetime membership. Certificates will not be ordered until your payment is received. Please include a copy of the pdf form called "Official Society Order" including the address of the submitting officer with your payment. Fill out the PDF version, print send two copies of the Society Order form, send one to Dr. Land and one to Dr. Pastorek.

Please send me the following four items (2-5 below).

2.  Completed PLU Membership card (National Office half) for each new initiate.  Please print the new member's name clearly on top line of the form in this order:  first name, middle name, and last name.
Don't forget to include the date of initiation and have the new member sign the card at the bottom! The address should be a permanent address (for students, use home address.) 

3.  Completed PLU The Register cards for each initiate. 

4.  One copy of the completed form called "Official Society Order" including the address of the submitting officer. Please fill in the PDF version, print and send two copies of the Society Order form with your payment.

5.  Email the Chapter Certificate Order Form to me using this spreadsheet. Clearly indicate the initiates in alphabetic order (first name, middle name, and last name), chapter name/school name and the initiation date.  Include one paper copy of the spreadsheet with your membership cards.

B.  To submit your chapter officers list, please use the Acrobat  PDF electronic format. Please email me the final copy (preferred) or you can fill it out, print and mail to me by snail mail. 

C.  To submit your chapter annual activity reports use the Acrobat PDF electronic format.  Please email me the final copy (preferred) or you can fill it out, print and mail to me by snail mail.
